Traxectoria Pilger foi coñecido por criticar fortemente a política exterior dos Estados Unidos de América, Australia e do Reino Unido, que segundo el estaban guiadas por unha axenda imperialista. Pilger tamén criticou o trato que recibían os aborixes australianos no seu propio país.
A súa traxectoria como director de vídeos documentais comezou con The Quiet Mutiny (1970), feito durante as súas visitas a Vietnam, e continou dende entón con preto de cincuenta documentais. Outros traballos relevantes son Year Zero (1979), sobre as consecuencias da fin do réxime de Pol Pot en Camboxa, e Death of a Nation: The Timor Conspiracy (1993). Entre os documentais sobre a temática dos indíxenas australianos destacan The Secret Country (1985) e Utopia (2013). No tocante á súa faceta como xornalista, destaca o seu traballo no Daily Mirror británico dende 1963 e 1986,Biography page, páxina oficial de John Pilger e como columnista habitual na revista New Statesman entre 1991 e 2014.
